Athlete Waiver *

In consideration of being allowed to participate in any way in Team Illinois Volleyball programs, related events and activities, the undersigned acknowledges, appreciates and agrees that: * 1) The risk of injury from activities involved in this program is significant, including the potential for permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist; and 
 * 2) I knowingly and freely assume all such risks, both known and unknown, even if arising from the negligence of the releases or others, and assume full responsibility for my participation; and 
 * 3) Iwillinglyagreetocomplywiththestatedandcustomarytermsandconditionsforparticipation.If however, I observe any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such to the attention of the nearest official immediately; and 
 * 4) I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, herebyrelease and hold harmless Team Illinois Volleyball; their officers, officials, agents and/or employees, other participants, sponsoring agencies, sponsors, advertisers and if applicable owners and lessors of premises to use to conduct the event (“Releasees”), with respect to all and any injury, disability, death, or loss or damage to person or property, whether arising from the negligence of the releasees or otherwise, to the fullest extent permitted by law; and 
 * 5) I will bring to the attention of owners/coaches, and be prepared for any special considerations due to participation in outdoor activities including but not limited to: 
- Allergies
- Bee Stings/Epi Pens - Sunburn
- Dehydration
- Diabetes 
 ATHLETES ARE PERMITTED TO TAKE WATER BREAKS WHENEVER THEY DEEM NECESSARY AND WILL BE GRANTED FORMAL WATER BREAKS AS WELL. TEAM ILLINOIS WILL DO IT’S BEST TO PROVIDE PROPER CONDITIONS FOR THE SAND COURTS, BUT WILL NOT BE LIABLE FOR ANY RELATED INJURIES OR DAMAGE. I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

Player Agreement *

I pledge to be responsible for my participation by following the Player Code of Conduct of Team Illinois Volleyball: • I agree to practice good sportsmanship at all times, to win without boasting, lose without excuse and never quit. • I agree to attend and participate in all scheduled games and practices when reasonable possible. • I agree to be aware of safety and will follow team and league rules to ensure safe play. • I agree to my coachʼs authority and will participate and communicate positively with my coach and teammates. • I agree to treat fellow players, opponents, fans and referees with respect. • I agree to exercise self-control at all times, refraining from foul language and setting a positive example for others to follow. • I agree to support and encourage my teammates, and to always try my best to try and keep a positive attitude. My signature verifies that I have read, understand and agree to abide by this Code of Conduct. Consequences may include removal from practice/game, suspension and/or removal from our league.

Parent Agreement *

I hereby pledge to provide positive support, care and encouragement for all participants, coaches, other parents and referees in Team Illinois Volleyball: • I agree to cheer, let the coaches coach, let the referees ref and let the players play. • I agree to respect the schedules of the coaches and teammates by having my player arrive on time for practices and games and will notify my coach if my player will arrive late or not able to attend. • I agree to be a positive role model and will not engage in public displays of anger. • I will promote good sportsmanship by respecting opposing fans, coaches, participants and referees. • I agree to respect coaching decisions regarding playing time, position and placement and will refrain from coaching any player. • I will not approach players or coaches before, during or immediately after games with anything other than support. All questions/concerns regarding players or coaches will be addressed the day after the game in an atmosphere conductive to problem solving. My signature verifies that I have read, understand and agree to abide by this Code of Conduct. I also agree to the consequences determined by Team Illinois Volleyball. I have also read and discussed with my daughter the Players Code of Conduct. Consequences may include removal from practice/game, suspension and/or removal from our league.

Media Release

I grant to Team Illinois Volleyball, its representatives and employees the right to take photographs and videos of me and my efforts in connection with the 2019/2020 club volleyball season. I authorize Team Illinois Volleyball, its assigns and transferees to copyright, use and publish the same in print and/or electronically. I agree that Team Illinois Volleyball may use such photographs of me with or without my name and for any lawful purpose, including for example such purposes as publicity, illustration, advertising, social media and web content.

Covid-19 Release *

I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing. I further acknowledge that Team Illinois Volleyball LLC has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19. I further acknowledge that Team Illinois Volleyball LLC can not guarantee that I will not become infected with the Coronavirus/Covid-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of myself and others, including, but not limited to, staff, and other players and their families. I voluntarily seek services provided by Team Illinois Volleyball LLC and acknowledge that I am increasing my risk to exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all set procedures to reduce the spread while attending my practices. I attest that prior to each practice: * I am not experiencing any symptom of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell. * I have not traveled internationally within the last 14 days. * I have not traveled to a highly impacted area within the United States of America in the last 14 days. * I do not believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19. * I have not been diagnosed with Coronavirus/Covid-19 and not yet cleared as non contagious by state or local public health authorities. * I am following all CDC recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19. I hereby release and agree to hold Team Illinois Volleyball LLC harmless from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the club, or that may otherwise arise in any way in connection with any services received from Team Illinois Volleyball LLC. I understand that this release discharges Team Illinois Volleyball LLC from any liability or claim that I, my heirs, or any personal representatives may have against the club with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from Team Illinois Volleyball LLC. This liability waiver and release extends to the club together with all owners, partners, and employees.
